Communications Department Equipment User Agreement:
The equipment and all the accessories must be returned on the date it is due. The patron will be notified by email if the item(s) is/are late. If the equipment is not returned after the email notification, collection will be referred to Simmons University Public Safety and/or may affect your ability to register for classes, graduate, or obtain copies of transcripts, or may be referred to the Dean of Student Life as violations of the Simmons University Honor Code.
By agreeing below, I understand that I will be held responsible for returning this equipment and all its accessories in the same condition as when checked out. I will be responsible for replacing or repairing any damaged and/or missing item(s).
